Instructions: How likely are you to doze off or fall asleep in the following situations, in contrast to feeling just tired? This refers to your usual way of life in recent times. Even if you have not done some of these things recently try to estimate how they would have affected you using the scale below.

  • 0 means that you would NEVER doze in this situation
  • 1 means that there is a SLIGHT chance of dozing in this situation
  • 2 means that there is a MODERATE chance of dozing in this situation
  • 3 means that there is a HIGH chance of dozing in this situation
